We use wound VACs in orthopedic surgery for large, deeper wounds in which we want to promote healthy granulation tissue. They do an especially good job at removing excess fluid and can help limit chances of infection. In general, you change the sponge every few days. Once the good granulation tissue has formed, the wound can then often covered with a split thickness skin graft.
